Admission Application Forms, PreK-1 Applicants

Application Forms for school placement are available in a PDF format. To apply for admission, complete the application forms listed below and return them to the Admissions Office together with all other supporting documents. All PDF forms need to be completed and emailed to admissions@aas-sofia.org. Please avoid large attachments (max total of 5Mb).Enrollment will not be considered until all of the required documents have been submitted to the school.

Application Forms PreK-1

1. Form A: Application For Admission. It should be completed and signed by the student’s parent or guardian, financially responsible for the student’s enrollment. No application fees is charged.
2. Form B1: Student Questionnaire, completed by the applicant or his/her parent.
3. Form C: Medical Form and give for completion to a certified doctor.
4. Form D1: Confidential Student Reference Form, to be completed by the applicant’s current teacher.
5. Previous school Report Cards for the past two years, translated in English. Please include all standardized tests/scores that you may have access to.
6. Provide copies of mother’s, father’s and student’s passport.
   * Provide Special Needs Report if applicable.
